Diagnosis

Hernia

Most hernias are from a defect (a hole) in the fascia (the strength layer of the body). They from either from congenital (present at birth) points of weakness, or at sites of prior incisions. These can then allow internal content to herniate (protrude) and in some cases get trapped in the hernia. Left alone they can slowly grow and could sometimes lead to ischemia (constriction of blood supply) to the content of the hernia-which becomes a life threatening emergency. It is best to treat the hernia earlier when it is smaller, and before and emergency arise to achieve a stronger repair and faster recovery.

Gallbladder

Gallbladder disorders are common, and most are from mobile stones that can form within the gallbladder. If these stones permanently obstruct the gallbladder it can lead to infection and at that point become an urgent surgical problem. In other cases, patients may have no stones but have dyskinesia or poor function of their gallbladder. These are rarely an emergency, but can cause severe pain that improves with the removal of the gallbladder.

Breast

Management of concerning, premalignant, and malignant breast lesions are complex and require highly individualized step wise approach. The goal of care is to help you understand your diagnosis, help you review treatment options, inform you of what is recommended based on national guidelines, and most importantly, help you choose a treatment the best fits your unique risks and goals of care. To optimize the success of your chose treatment and follow-up, a multidisciplinary approach is used by coordination of your surgical care with the radiologist, pathologist, oncologist, and radiation oncology, as well as plastic surgeon for possible reconstruction.

Reflux and Hiatal hernia

GERD or reflux is an increase in acidic gastric juice in the esophagus that can cause discomfort, damage to the esophagus, as well as pulmonary symptoms. While most patients can have good symptoms control with medication, some cannot get sufficient control. If a hiatal or paraesophageal hernia is present, which allows the stomach to rise into the chest, these symptoms can be exacerbated and surgical treatment becomes more strongly indicated.

Colon and other bowel surgery

Benign disease of the colon such as stricture, fistula or diverticular disease, as well as malignant lesions of the colon require surgical resection. These are often best addressed on elective basis, before they become an emergency due to perforation, infection, or spread of the disease.

Thyroid disease

Some thyroid nodules can be followed with imaging and biopsy. However, when biopsy revels concerning findings a partial or total thyroid resection is recomended to further evaluate or treat the nodule. Other patient develop multitude of lesions causing the thyroid to enlarge (called thyroid goiter) and resection is then recommended or preferred over monitoring.
